Blood Pressure Monitoring
Who to see: Nurse / Health Care Assistant
Advice: If you have high blood pressure we will offer you an annual appointment to review this which will in the month of your birthday. We like to check three main points when we see you
1) Your average blood pressure reading
2) A blood test to check your kidneys are ok (if you are on blood pressure medication)
3) Your height and weight and a lifestyle questionnaire
You need to book a blood test appointment with the Healthcare assistant.
If all your results are within range, we will see contact you again in one year’s time. If there are any problems with your results, we will contact you.

Advice: Clinics are held daily at various times. Appointments for blood tests MUST have been requested by your doctor or other health care professional. If you are in doubt please call to speak to one of our nurses.

Cervical Smears

Advice: You will be invited when your test is due. Once you are invited, please make an make an appointment (ideally day 14 from first day of your period).
Chest Infections

Children's Immunisations

Chronic Heart Disease
Who to see: Nurse or GP
Advice: If you have heart disease you will be offered a ‘Care and Support planning’ method of consulting. This occurs in two appointments and will be in your month of your birthday:
1) Appointment one is with the Healthcare assistant who will check your blood pressure, blood tests and a number of other measurements. One the results of this appointment come back to the surgery we will pass them on to you.
2) Appointment two is with a Nurse or GP who will focus on your current health and wellbeing goals to improve your health

Advice: Antibiotics are not needed for the common cold as colds are caused by a virus.

online consulting. If however you are an adult and you have had a change in bowel habit for more than 2 weeks which is not settling you should see a GP.
